drill a second hole directly adjacent to the first hole and then use an ice saw to connect them and form one big hole?

You just drill the second Hole adjacent to the first to expand the size. The second hole became part of the first hole. It probably only needed to be a bit bigger. Yea…doing that while her arm was down the hole, there could be a possibility for injury, but very small.
The fish died so I am guessing the extended hour long fight is what killed it. By the time she grabbed it the fish probably didn’t do any fighting any more.
