You do put a 30.00 in the blank.
Only by mail or in person. If you mail, be sure to go to the post office and get an international stamp.

I have a question about the RABC form, do i enter 30 on the line with the red arrow?
Also, RABC can only be done via Mail or in person, correct, there is no online form?

Do as Moose stated. Your credit card issuer will do the conversion which has been about $.75 cdn to $1.00 usd. Try to use a card w/o a foreign transaction fee and save yourself 1-3%.
